Does a parent need to be present for a student’s appointment?

Sometimes.

For sports physicals, well-child exams, and illness visits, we require a parent or legal guardian to be present at the appointment or available by phone.

For mobile vaccine visits in the high school or middle schools, parents or legal guardians do not need to be present. As long as we have a consent form signed, we can see the student during school and send them right back to class.

Each appointment and student is unique. Always feel welcome to reach out to clarify who should be at the appointment.

How do I change or cancel an appointment?

Is my student’s information shared with the school or health office?

The Bear Care Clinic operates separately from the district. Just like any other clinic or doctor’s office, we cannot share health information with the school or health office without your permission.

On your consent form, we will ask your permission to share vaccine information with the health office to help ensure the student has the correct vaccines needed. We may receive school schedule information from the school if we need to call a patient from class for their scheduled appointment.

If my student is in the high school, do they need a pass for their Bear Care appointment?

Yes. Just let the front office know they have an appointment. The student will be given a pass to come to the clinic. We will give the student a pass to go back to class after the appointment.
