Minnesota law requires primary care and pediatric clinics to post the following information annually for the top 25 most frequently billed Current Procedural Terminology (CPT) codes over $25, including the ten most commonly billed Evaluation and Management and Preventive Medicine services. This list is not inclusive of all services offered at our clinic.


The pricing information listed in the document below does not reflect the amount(s) each clinic patient will pay for the services listed. Your actual fee may be higher or lower depending on many factors, including, but not limited to, your provider’s treatment plan, actual services rendered, complications, and the details of your insurance coverage, if any. Additionally, your deductible, coinsurance, and copay all affect what you’ll pay.

Charges represent the standard amount Rise Up Health Clinics bills for a service. For most patients, Rise Up gets paid an amount below the listed charge. Patients covered by commercial health insurance: Your health insurance company has negotiated a contracted rate for each service. Your health insurance company’s negotiated price might be higher or lower than the average commercial payment amount listed above. Please contact your insurance company with any questions you have about your plan.
